Analysis of the Correlation between Structured Education Programand Incidence of Readmission (30 days) of Patients with HeartFailure after Discharge at Siloam Hospitals Lippo Village

The majority of people with heart failure have chest pain, coughing, edema in both legs, exhaustion, easy sleepiness, shortness of breath during activities, and trouble sleeping. Most patients also don't know about essential at-home care, like when to weigh themselves, when to limit their intake of fluids, and which medications, like blood thinners, should be taken with green veggies. Poor self-care has been related to patient readmissions and indicates unfavorable health outcomes. to analyze the impact of a Structured Education Program conducted by a heart failure education team led by a Nurse on readmission (30 days) after discharge in heart failure patients. The study is a quantitative research observational study that used a consecutive sampling with a total of 96 samples of heart failure patients who received structured educational visits from the heart failure team in the Inpatient units. The results of this study indicate a correlation between the team’s provision of complete structured education and the incidence of heart failure patient readmission (30 days) after discharge (p-value 0.012). The Hospital Readmission rate (30 days) after discharge of patients with heart failure was 9.52%, which decreased to 4.55%. This study has demonstrated how the heart failure team's adoption of the Structured Education Program has influenced patient behavioral aspects linked to preventing readmissions, such as poor dietary choices and medication adherence.
